Front desks in dental offices are stretched. In the ADA Health Policy Institute’s survey for the first quarter of 2026, one in five dentists said they don’t have enough administrative staff, and more than half of the dentists who tried to recruit administrative staff called it very or extremely challenging. Meanwhile the phone keeps ringing: new patients, rescheduling, insurance questions and toothaches. An AI receptionist can take a large share of those calls. Here is what it does well, where a person should stay in charge and what to check for HIPAA before you start.
What an AI receptionist does in a dental office
- Answers every call, including at lunchtime, after hours and when the whole team is chairside.
- Books, moves and cancels appointments directly in your schedule when your practice management software offers an integration, or collects the request for your team to confirm.
- Answers routine questions from your own information: hours, directions and parking, accepted insurance plans, new-patient forms and what to bring.
- Recognises urgent cases, such as pain, swelling or a knocked-out tooth, and follows the rules you set: an emergency slot, a call-back or the on-call number.
- Takes complete messages with name, date of birth, reason for the call and best call-back time, so nobody has to call twice.
- Speaks your patients’ languages, for example English and Spanish, on the phone and on WhatsApp or SMS.
What should stay with your team
- Clinical advice and diagnosis. The assistant gives practical information, never a diagnosis or treatment advice.
- Medical emergencies. Callers with a medical emergency should be told to call 911; that rule belongs in the script from day one.
- Billing disputes and complex insurance questions that need someone to look into the patient’s account.
- Anxious or upset patients, who should be able to reach a person at any moment.
HIPAA: what to check first
Calls to a dental office contain protected health information: names, appointments, symptoms and insurance details. Under HIPAA, a service that creates, receives, maintains or transmits that information for your practice is a business associate, and HHS guidance makes clear you need a business associate agreement (BAA) with it, also when it is a cloud service. Before you sign, check:
- that the provider signs a BAA, and that the services it relies on, such as telephony, transcription and the AI model, are covered too
- where call recordings and transcripts are stored, who can access them and how long they are kept
- that the assistant asks only for the information it needs to handle the call
- that access is logged and that you can export or delete data when you stop
Reminder calls and texts: get consent
If the assistant also calls patients, US telephone rules apply. In February 2024 the FCC ruled that AI-generated voices count as artificial voices under the Telephone Consumer Protection Act, so calls that use them need the prior express consent of the person called, unless an exemption applies (FCC declaratory ruling). Collect consent for calls and texts when patients register, and ask your counsel how the rules apply to your appointment reminders.
How to roll it out
- Count your calls. For two weeks, let the front desk note the reason for every call. The most frequent reasons are what the assistant handles first.
- Start with after hours and overflow. The assistant answers when nobody else can, so your team stays in control during the day.
- Connect the schedule. Check whether your practice management software offers an integration; if not, start with booking requests your team confirms.
- Agree the urgent-case rules with your dentists: which symptoms get a same-day slot, a call-back or the on-call number.
- Review conversations weekly in the first month and improve the answers, then add more tasks.
What it costs
The costs depend on call volume, the integrations and the languages. We break down set-up and running costs in our AI receptionist cost guide, and you can see what we build for practices on our page about AI agents for clinics and healthcare. A clear practice website with online booking takes pressure off the phone too.
